We must first receive your completed personal sports profile questionnaire
and payment before we can begin working on your sports resumes. Your
sports resumes will be shipped to you within 5-10 business days.
If urgently required, your sports resumes can be sent to you overnight
once completed (See your order form for more information). Otherwise
you will receive your sports resumes within 5-10 business days of completion.
Our RATES are very affordable. The required
information is obtained from you by mail. It is very easy and it
supplies you with a tool that can aid you in obtaining your vision.
Note: All photos
you submit to be placed on the back of your resume will be returned to
you along with your completed resumes. See your order form for more
information regarding your personal sports action photos.

Sports Dreams will send you a form (personal profile questionnaire)
that is designed to supply us with the information we need to complete
your sports resumes. Once completed send the profile questionnaire
along with the other specified information back to Sports Dreams.
Upon receipt of your questionnaire, your personal sports action photo(s),
your order form, and full payment, Sports Dreams will begin preparing your
sports resume. Completion times will vary depending on the information
and photos received. If needed, you may call or e-mail and receive
an approximate time frame for completion. Currently payment is
accepted by check or money order only. In the near future, we will
be accepting payment by credit card.
You have a couple of options as to how you would like your sports
resumes shipped. Your order form will detail all shipping and
handling cost. These costs must be included in your full payment.
Sports Dreams will not begin preparation of your sports resumes until full
payment has been received. You may be notified by e-mail when your
payment has been received if you elect to do so (See order form).
Note: The above stated postage and handling costs are for clients
within the contiguous United States. At this time we do not service
any customers outside the contiguous United States. For more information
regarding sport resume delivery outside of the United States, please send
an e-mail to customerservice@fundamentalsbasketball.com.
Order forms will arrive with the personal profile questionnaire
that you will receive in the mail. The order form will provide you
with the necessary information needed to order your sports resumes.

and money orders with your order form to:
